The process

Four steps from first call to first visit

No paperwork mountain, no provider runaround.

  1. 01

    Phone call

Start with a call. Tell us what's going on and what kind of support you're after. No forms first. The conversation is free and there's no obligation.

  2. 02

    Nursing assessment

A registered nurse visits you at home (or wherever suits) to understand your health needs, your goals, and what's already working. Clinical depth from day one, not a checklist.

  3. 03

    Your support plan

We write a plain-English plan that maps what we'll do, who will do it, and how often. You read it, ask questions, sign when you're happy. No jargon.

  4. 04

    First visit

Your nurse or support worker arrives and care begins. Same people every time, no rotating roster.

What to have handy

Nothing here is essential. The more we know up front, the quicker we can get to the right answer.

  • Your current NDIS plan (if you have one)
  • A phone number and email we can reach you on
  • A rough sense of what kind of support you're looking for
  • Any recent hospital or specialist letters (helpful, not essential)

Missing something on this list? That's completely fine. Start with whatever you have and we'll help you gather the rest along the way. There's no pressure and no obligation to go ahead.

Ready when you are

The fastest way to start is a phone call. You'll reach a real person on the Central Coast, not a call centre.

Common questions

Before you call, you might be wondering.

Do I need an NDIS plan to get started?
No. If you don't have a plan yet, we'll talk you through how to apply and what to ask for. If we're not the right provider for you, we'll tell you and point you to who is.
How long does onboarding take?
Usually one to two weeks from your first call to your first visit, depending on your needs and availability. We can move faster in urgent situations, especially for post-hospital support.
Is the initial nursing assessment free?
Yes. The first conversation and the initial nursing assessment are free, with no obligation to proceed. We want you to feel confident before you commit.
